$200.00 Advance Deposit is Required – make check payable to Local 808.
I UNDERSTAND THAT IF IT IS FOUND THAT THE ABOVE INFORMATION IS UNTRUE, AND RENTAL IS FOR OTHER THAN WHAT I STATED, MY DEPOSIT WILL BE FORFEITED IN FULL.
I HAVE READ THE RULES FOR THE USE OF LOCAL 808 UNION HALL AND UNDERSTAND THAT I AM SOLELY RESPONSIBLE UNDER THE STIPULATIONS OF THOSE RULES.
I ALSO UNDERSTAND THAT ALL FUNCTIONS OF LOCAL 808 HAVE PRECIDENCE OVER MY USE OF THIS UNION HALL.
