Every employee, having seniority, will receive group hospital, surgical and dental indemnity insurance benefits for themselves and their dependents as provided under the Whirlpool Employee Benefit Plan Group Policy Number 06957. Such plan provides medical and dental insurance benefits for employees and (upon employee´s election in accordance with the Plan) the employee´s spouse and dependents and may or may not be on a contributory cost basis. The Master Insurance Policy sets forth the provisions of the Insurance Plan. A summary of benefits and other provisions of the Plan are outlined in the Summary of Coverage. The Summary of Coverage contains the principal benefits provided by the Plan, but is not intended as a complete listing of all coverages provided.
